A leading integrated care services organization is seeking an experienced Director of Clinical Care Coordination to provide leadership and strategic direction across clinical care coordination programs. The role focuses on strengthening patient transitions, improving care continuity, supporting interdisciplinary collaboration, and enhancing coordinated healthcare delivery, with direct management of approximately 8 to 12 clinical care coordination team members. Reports to the Vice President of Clinical Operations. Compensation: $145,000 to $175,000 base salary plus performance incentive and benefits.
What you'll do
- Provide leadership and direction across clinical care coordination initiatives
- Guide the development and ongoing improvement of care coordination programs
- Establish effective workflows that support consistent and coordinated patient care
- Monitor care coordination activities and identify opportunities for operational improvement
- Support the implementation and maintenance of clinical policies, procedures, and quality initiatives
- Support effective transitions between healthcare settings and services
- Promote continuity of care throughout the patient journey
- Help identify and address barriers that may affect successful patient transitions
- Support patient engagement processes and coordinated follow-up
- Collaborate with nurses, case managers, physicians, and operational teams
- Facilitate communication between clinical and operational stakeholders
- Support cross-functional healthcare programs and initiatives
- Directly manage approximately 8 to 12 members of the clinical care coordination team
- Provide leadership, guidance, and professional development support to team members
- Help establish clear expectations and performance objectives
- Contribute to the development of a strong, patient-focused clinical culture
- Monitor care coordination workflows and service delivery processes
- Participate in clinical quality initiatives and policy development
- Use operational and clinical information to identify areas requiring improvement
- Support continuous improvement across integrated care services
What we're looking for
- Strong professional experience in clinical nursing, care coordination, or case management
- Demonstrated experience in clinical operations and healthcare service delivery
- Proven team leadership or clinical management experience
- Experience with interdisciplinary healthcare collaboration
- Background in patient liaison or patient support services
- Ability to work effectively across clinical and operational functions
- Strong communication with a range of healthcare professionals and stakeholders
- Experience in care transition management and quality improvement
- Resume/CV outlining clinical, care coordination, case-management, and leadership experience
